AAFS (Station) Grant Request Form

This form is for all Adopt-A-Fire-Station (AAFS) requests and departmental requests. Please note:

• If you are asking for more than one item, please fill out only one application and include/combine every item in this request.
• If you are ordering from vendors that allow you to share your cart (ex: Home Depot), please use the option and include the link in the application. 
• Reimbursements will not be allowed. 

The application will be processed promptly if your station/section has enough funds in its account. In this case, an LAFD Foundation representative will reach out to confirm the request details are correct. If no response is received from the station in 72 hours, the request will be declined/withdrawn. If this request is granted, please either obtain and submit an F-225 form through proper LAFD channels, or print out this completed application and submit it as your F-225 form.
